FPNO Monthly Reporting

This form should be submitted within 48 hours of each FPNO event. Before submitting, be sure to have scanned copies or photos of Incident Reports, Mandatory Reports, and any photos from the event available for upload.

  • Please do NOT include the amount your FPNO budget pays for during the event. Instead, consider the costs of things that your event site covers outside of the budget such as facility rental, donated meals. Also consider discounted or donated services such as special guests, special activities, supplies, etc. This is needed for FPNO government reporting.

        • Think about any moments during FPNO when you observed a positive change in a child because of a meaningful connection. Is there a story of a volunteer who was positively impacted by a child? Any feedback you may have received from a foster family about a difference made because of FPNO for their child/children in care? Please include any pertinent stories from the night.
        • If you have photos from your FPNO event taken by the Site Coordinator(s) or designated volunteer, where minors are unidentifiable, please upload those here. We love sharing this inspirational work you’re doing with our partners at ODHS and to encourage others statewide!
